Medicare ERA Enrollment

  • Go to the CMS website or your Medicare MAC portal.
  • Download and complete the CMS-855 or MAC-specific ERA/835 form.
  • Submit the form through your MAC's portal or as directed.
  • Wait for approval; you will be notified when ERA is active.
