  • Visit the EDISS website to access enrollment forms.
  • Download and complete the ERA/835 enrollment form for your payer/state.
  • Submit the form to EDISS via their portal or as instructed on the form.
  • Wait for confirmation from EDISS regarding enrollment status and next steps.

Visit the link below and complete the required enrollment:
a. Company Name – Availity LLC
b. Trading Partner Id – CH00033
c. Do not select Availity LLC to ‘Manage Vendor’ account.
