Enrollment Instructions:
An agreement must be completed for every billing TPI number that wants to receive ERAs.  The Availity Production User ID (Submitter ID) is 146206501.  The submitter ID will go in the Use Production User ID field.  If providers are already getting ERAs through another submitter ID and want to switch to Availity, then mark CHANGE Production User ID on the form and include the submitter IDs that were previously registered.  If this is the first time registering for ERAs then check Set up INITIALLY and include 146206501.
